Nephrology services
- Chronic kidney disease care — Diagnosis and staging, slowing progression, anemia and bone disease management, dietary guidance, and transplant or dialysis planning for CKD stages III through V.
- Hypertension management — Evaluation of high blood pressure including secondary and resistant hypertension, home monitoring review, and medication adjustment.
- Dialysis care — Hemodialysis and peritoneal dialysis management, access planning and referral, adequacy monitoring, and coordination with dialysis units.
- Proteinuria and glomerular disease — Workup of excess protein in the urine, blood in the urine, and swelling, with treatment of the underlying kidney condition.
- Electrolyte and fluid disorders — Sodium, potassium, calcium, and acid-base abnormalities, plus management of fluid retention and edema.
- Acute kidney injury — Inpatient and outpatient evaluation of sudden decline in kidney function, including post-hospital follow-up.
Medicare care management programs
- Chronic Care Management (CCM) — for patients with two or more chronic conditions expected to last at least a year.
- Principal Care Management (PCM) — focused management of one complex condition, such as advanced chronic kidney disease.
- Remote Patient Monitoring (RPM) — blood pressure and weight readings reviewed by your care team.
About Dr. Chen
Kevin Chi-Wen Chen, MD is board-certified in Nephrology and Internal Medicine and graduated from the University of Minnesota Medical School in 1996. He works with primary care providers, hospitals, and dialysis facilities across southern Illinois, with affiliations at Deaconess Illinois Medical Center, Pinckneyville Community Hospital Specialty Clinic, and Marshall Browning Hospital Specialty Clinic.

- Is there a cost for Medicare care management?
- Advanced Kidney Institute bills your insurance for this service. These services are subject to the usual Medicare deductible and coinsurance applied to physician services. Many patients with supplemental coverage owe nothing.
- Can I enroll in care management with more than one practice?
- No. Only one provider may furnish this service at a time. Please tell us if you have already entered into a similar agreement with another provider or practice.
- Can I stop care management at any time?
- Yes. You may revoke the agreement at any time in writing, effective at the end of that month.
- Do I need to come into the office for care management?
- No. Care management happens by phone and other non-face-to-face communication, in addition to your regular visits.
